Jim's death and reincarnation

My husband and I are new to this show. We have been watching the old episodes. We are so lost about Jim dying and coming back. Does Melinda only physically see him as "her" Jim and does everyone else physically see him as Sam? We are catching up on old episodes and watching the new--which now we are completely confused about Jim!

I too just started watching this series. I saw an episode where Melinda was marrying Jim/Sam right after he figured out who he is and she was pregnant and Jamie Kennedy was in it. Then, tonight I saw an episode with Jay Mohr where she is calling her husband Jim and they are disagreeing about whether or not to have children. I am totally confused.

Jim and Melinda decided to try for children. Then Jim gets killed, but as a ghost decides to jump into Sam's body (who has just died and gone into the light). Once in Sam's body Jim gets amnisure and doesn't remember Sam or Jim. He eventually remembers who he is and they find out that Melinda is pregnant, by Jim before he died. Jim and Melinda re-marry and everyone sees him physically as Sam, but they kept the same actor because he's still Jim on the inside. Sometimes you get a shot of Jim's reflection and it will be the actor who played Sam in the reflection. All very confusing to someone not watching them in order I'm sure!

How did Sam/Jim get to be a doctor in only 5 years? Sam was an architect and to the world he is Sam. I don't think a person with no medical training could become a doctor in only 5 years. This is very troubling to me.

In the most recent episode, a worker comes into the hospital records room and calls Jim "Jim". I hate lazy writing. He is supposed to still be Sam, with only Melinda and friends knowing that it is Jim occupying Sam's body. Or do the writers just assume that the general population is too incompetant, and with too short of a memory to notice? It is annoying to see people with ivy league degrees getting paid a lot of money to be so careless.
