season 7 is a bit unusual because it has two totally
unrelated major storylines going on at the same time. First you have
the Leviathans and the newly discovered fact that they're allegedly
trying to cure cancer to help mankind. Then you have Sam's deteriorating
mental state as Lucifer taunts his inner mind.

It's possible that the two will have something to do with one another in the end (like the angel civil war and the search for Purgatory in season 6), but for now, they feel totally separate, and this week is all about Sam.
In this clip, we see that Sam is in a hospital, ready to accept his inevitable fate as Lucifer mocks him from the sidelines. Dean, however, is not willing to give up so easily.
Not only does this episode mark the return of Misha Collins (foreshadowed as Dean and Sam discuss Cas in this scene), but there's also a shout-out to the season 1 episode "Faith."
Will Dean be able to help cure Sam (possibly with an assist from Cas)? Or is Sam really doomed this time? And does any of this have anything to do with the Leviathans?
