As Michael prepares himself to head to the hospital, Mahone stops him and tells him of a guy that he worked with who was in a similar situation.  He described the extreme amount of stress the man was under, worried that he wouldn't make it out alive and therefore, a lot of criminals would be on the streets because he wouldn't be there to stop them.  Mahone promises Michael that, no matter what, he'll carry out the plan that has been set forth to take down The Company.

T-Bag realizes that Trish Ann is refusing to take off her jacket because she's a fed who's hiding a gun under it.  He panics to Gretchen, that he won't see the money that he was promised for teaming up with her.  Th two call a meeting with the Asian man that has been showing up at Gate looking for Cole and Scylla.  Don meets with Michael to let him know that Gretchen was unable to get the 6th card.   At the last minute, Michael decides not to go through with the surgery, unable to bear the fact of losing another man.  Sara instead gives him an injection that should prevent any seizures in the next 3 hours and insists that he go to the hospital the minute this is over.

The General lets Lisa know that Gretchen made a move on his card and demands that Scylla be immediately moved. T-Bag and Gretchen fake a phone call with a fake meeting place to set Trish Ann up. Michael heads into Gate with Sucre, Lincoln, and Mahone and insists on absolute silence from the entrance point on, for safety reasons. The men work side by side, in silence, following every instruction that was given to them by Michael earlier in the day.  Little by little they dig through the concrete wall, inching closer to Scylla.
