Growing up in the suburbs of Chicago, Sara Tancredi wanted to be a doctor and work in a hospital. However, after earning a medical degree, Sara's hopes in becoming a doctor were jeopardized by her constant drug abuse. Coming clean with the help of Narcotics Anonymous, she knew that she must follow a more humanitarian path and decided to work as a doctor at Fox River Penitentiary, against her father's wishes.

Things began to get complicated for Sara when inmate Michael Scofield entered her life. Their relationship started out with the usual doctor-patient dynamic, but after a couple of what seemingly innocent and harmless flirting, Sara found herself falling for Michael’s charm, intelligence and sincerity. As their relationship blossomed, Michael, knowing that she was the daughter of Governor Frank Tancredi, asked her to help his brother Lincoln, who he believed to be innocent. Initially, she felt reluctant to do so, but when she finally approached her father, her request was flippantly dismissed. Nevertheless, her love for Michael drove her to help him and other inmates escape from Fox River.

Confused and guilty, Sara reverted back to her old habit and began abusing drugs once again. When she woke up from a potentially fatal drug overdose, she decided to go back to rehabilitation and patched things up with her father. However, Michael found a way to contact her and eventually, she grew convinced of his intentions and the sincerity of his feelings. Unknowingly, she befriended Agent Paul Kellerman, who was using her to get to Michael. Her father was the one who revealed Kellerman’s true identity, and after being presented with new evidence in favor of Lincoln, was found dead in his den. Sara knew that her father had been killed, and soon became the next target. She reunited with Michael in New Mexico, only to be chased by Agent Alexander Mahone. After narrowly escaping Mahone, Sara was abducted by Agent Kellerman, who tried to get her to talk about the new evidence by torturing her. Ultimately, she was able to escape Kellerman as well, but when she was on her way to meet Michael and Lincoln in Panama, the FBI arrested her.
