During the discovery of the Oceanic Flight 815, spiritualist Miles Straume was visiting the Inglewood, California residence of Mrs. Gardner, whose son died in the house. She wanted his spirit exorcised from the house, and Miles agreed to talk to the spirit for a fee. He was then paid $200 in advance by Mrs. Gardner, and he proceeded in communicating with the boy's spirit through a ghostbuster device. However, he was also able to locate a stash of money and drugs which was kept hidden in the boy's room. Miles took the cash and left the drugs, later on refunding Mrs. Gardner $100. Miles was then recruited into a team led by Naomi Dorrit that was sent to the island aboard the Kahana freighter.

After the helicopter encountered some electrical problem, Miles parachuted off it to land on a rocky outcrop near the north side of the Island. Later, he was found by Jack Shephard, Kate Austen and Daniel Faraday as he appeared to be incapacitated. However, he surprised them by holding a gun at Jack, who tried to examine him. Miles accused the survivors of killing Naomi, and demanded that he be taken to her body. Later, they found her body and Miles was convinced that she was not killed by the survivors.

Jack then told Miles that they were covered by their friends with firearms. Miles dropped his gun when Sayid Jarrah and Juliet Burke came out of the jungle firing their guns. Miles then questioned Sayid about who he was and when Miles heard from Frank that Juliet was a native of the island, Miles demanded to know the whereabouts of the person in the photo that Miles held up. Miles said he came to the Island to find Ben Linus, the man in the photo.

Miles, Sayid and Kate then headed to the Barracks to rescue Charlotte Lewis. However, an ambush by John Locke and Sawyer Ford forced Locke and Sayid to trade Miles for Charlotte. While at the Barracks, Miles used Kate to get to Ben. Miles added that he wanted $3.2 million in exchange for telling his employers that Ben is dead. Kate responded by saying that Charlotte knew Ben was alive. Miles said he'd take care of Charlotte. Kate then tried to lead him to the boathouse, but Miles told her that he knew what she did and who she really was. Locke then appeared and brought him to the boathouse. A day later, Locke sticks a grenade into Miles' mouth and pulled the pin, forcing Miles to keep his mouth shut.
