Meredith and Cristina watch Derek and his friends laughing and celebrating Archer's successful de-worming. Meredith rues the fact that she doesn't really know this laughing, singing, guitar-playing social Derek. Cristina is all, "So, I don't know Owen." Owen finds Cristina and explains that he calls his mom once a week, but pretends that he's still in Iraq. He can't bear to tell her that he's back because she'll just look at him like he's a ghost, just like Beth does. The only time Owen doesn't feel like a ghost is when he's with Cristina because she sees the real him.

Ever the perceptive one, George (T.R. Knight) observes throughout the intern game that Sadie doesn't know a damn thing about medicine. He offers to help her get up to speed because at this point in her training, she should be able to diagnose patients and, you know, do things. She huffily walks away, saying that she doesn't need a tutor, but George calls after her that she could hurt someone seriously. Later on, he gives her the ultimatum that either she turn herself in to Richard for being a liability with patients, or he will. Sadie chooses to quit the program altogether because she would rather be known as a quitter than as a cheater. Apparently, she faked her way through medical school and was hoping that if she got into Seattle Grace, that Meredith would be able to keep her afloat. (Hrm, perhaps another reason why this hospital is slipping in the rankings?) Sadie wants to run away to Europe with Meredith like they did after medical school, but Meredith reminds her that she has a job, and actually wants to be a grown-up. So, I guess that's the end of Sadie. Her ineptness totally appeared out of nowhere, huh?

Lexie and Mark have just about the cutest exchange, in which Mark very professionally congratulates her on winning the intern game. Addison watches all this and deduces that they are sleeping together. She almost outs them to Derek, but Mark saves himself by blurting out that Derek is planning to propose to Meredith.

Derek finally shows his face to Jen and promises to schedule her for first thing in the morning. But she winsomely begs for him to do it now please, so he relents. In the OR, he gently mocks the doomsday sucker-wielding Lexie, but interrupts himself mid-mock when he nicks the aneurysm. He manages to tie everything off and patches Jen up, though.

At Joe's bar, Addison thanks Derek for being a god, but says that he's going to have to become tiny again and go back into his little box. Everyone sings an old song that Derek wrote for Addison in medical school. They drink and celebrate, but Derek gets paged because Jen is having complications. The whole gang follows him to the hospital, thinking that it's Archer who is having complications.

Alex tends to Jen who is having trouble breathing. Notwithstanding his little bout of temper with her earlier (he yelled her for being whiny and needy), his bedside manner has improved immensely since the beginning of Grey's Anatomy. He's confident, has a soothing voice, and he's knows what he's doing.
